IOM 2009 guidelines based on pre-pregnancy BMI
How Pregnancy Weight Gain Is Calculated
Recommended pregnancy weight gain depends on pre-pregnancy BMI, calculated as weight (kg) ÷ height (m)² before conception. The Institute of Medicine (IOM) 2009 guidelines define total gain ranges by BMI category: underweight (BMI < 18.5): 12.5–18 kg (28–40 lbs); normal (18.5–24.9): 11.5–16 kg (25–35 lbs); overweight (25–29.9): 7–11.5 kg (15–25 lbs); obese (≥ 30): 5–9 kg (11–20 lbs).
Gain is not linear across pregnancy. The first trimester (weeks 1–12) typically adds 0.5–2 kg total — often minimal due to nausea. Second and third trimesters add weight at BMI-specific weekly rates: underweight 0.44–0.58 kg/week, normal 0.35–0.50 kg/week, overweight 0.23–0.33 kg/week, obese 0.17–0.27 kg/week. This calculator models cumulative min/max curves across all 40 weeks.
Enter pre-pregnancy weight, height, and current pregnancy week to see recommended gain at that point, total gain range, and a chart with shaded min–max band. The dot marks your current week on the recommended curve. The week-by-week table lists cumulative gain at each gestational week.
Weight gain supports fetal growth, placenta, amniotic fluid, increased blood volume, and maternal tissue expansion. Insufficient gain associates with low birth weight risk; excessive gain with gestational diabetes, hypertension, and postpartum retention. Individual needs vary — twins and multiples require higher gain not modeled here.
Track trends with your prenatal care provider rather than fixating on weekly numbers. Sudden rapid gain, loss, or concerns about eating should prompt medical consultation. This calculator provides general IOM guidance, not personalized medical advice.

Frequently asked questions
It depends on pre-pregnancy BMI. Normal BMI (18.5–24.9) targets 11.5–16 kg (25–35 lbs) total. Underweight women need more; overweight and obese women less.
The fetus is tiny in early pregnancy. Most first-trimester gain is maternal tissue and fluid. IOM recommends only 0.5–2 kg total in weeks 1–12.
Discuss with your prenatal provider. Ranges are guidelines, not strict limits. Focus on nutritious eating and appropriate activity rather than dieting during pregnancy.
